Transcript: A Master's degree in an appropriate field, is required of all applicants. The degree must be in addition to a bachelor's degree. All degrees must be from accredited institutions. Request one official transcript from each post-secondary institution (college or university) that you have attended. Transcripts should be for both undergraduate and graduate. Personal Statement: Students should attach to the application a statement of academic and professional goals. Recommendation Letter: In your CollegeNet application on (page 5) is a space for you to add your recommender’s names and email addresses. When you submit your application they will receive by email a recommendation form to respond and will be able to add a letter on their letterhead. They will then click to send which will come to me electronically. Other Document: A copy of passport. Copy of diploma with English translation for degrees earned outside of the United States. Official / original transcripts with English translation.
