Recommendation Letter: Print and prepare three copies of the Evaluation of an Applicant for Admission to Graduate Study form and give one to each of the three individuals who has agreed to write letters of recommendation on your behalf. For each recommendation, the form must be signed and submitted directly by the recommender, along with their letter of recommendation (if separate), to the graduate program to which you have applied.
