Transcript: Official transcripts from each post-secondary institution attended are required. Curriculum Vitae: A resume of your educational background, work, and coaching experience is required. Personal Statement: A two-page typed essay that includes information about your background and prior experience, your professional purpose and career goals, and how the Coaching Education program can help you to fulfill your goals. Recommendation Letter: Three letters of recommendation using the recommendation form provided by the Graduate College. At least one reference must be from someone who can attest to your qualifications in your chosen field and at least one must be from an academic faculty member who can support your ability to produce successful academic work at the graduate level. Please be certain to identify your intended concentration before giving the form to the person providing the reference. Financial Support Form: Submiting a Financial Support Form with the application is not required, but it is recommended. The United States Citizenship and Immigration Services require that Ohio University establish that students have sufficient funds for the first year of study before a Form I-20 or DS-2019 Certificate of Eligibility can be issued. Having a complete Financial Support Form on file at the point you are admitted will speed up processing of your Certificate of Eligibility.
